How to Motion to Strike?

This is the boiler plate motion to strike that I found.
We have the "charges" against us and we have this massive CPS Report - a HUGE PILE OF SMEARING LIES ...

1) Do a motion to strike against the entries in the CPS REPORT?
2) Against the Charges?
3) Is it one motion to strike per entry in the CPS REPORT (if that is what we file motions against)
4) in the section: "[specify recognized grounds for motion, such as:
the complaint is not verified as required by __________ or
plaintiff has not filed a certificate of merit in this action as
required by __________ or the complaint is not drawn in
conformity with Rule 201 of the California Rules of Court in
that __________]." - is there any examples of this available? I've searched high and low for example Motions to Strike with respect to CPS and have found none.
